What should I do if I want a blood sample to be taken at home?

• Check that your doctor has written the words «sample to be taken at home».
• Call us on 488 288-1. Telephone calls are personally answered between 7 a.m. and 7 p.m. outside these times you can leave your request on our voicemail system. You can also send an e-mail via our web site under the button «contact».
• Ask for a blood sample to be taken at home.
• Give your surname and forename or that of the patient, if the patient is a relative (child, parent, etc.)
• Give the patient’s insurance number. This comprises their date of birth in reverse order plus three other figures.
• Give your address stating: the house number and street name; the locality; the floor number in the case of a block of flats and the name on the doorbell if different from the patient’s name.
• Provide a telephone number where you can be contacted in an emergency.
• If it is a repeat prescription indicate whether the original prescription is held by you or the laboratory.
